This privacy statement describes how Priority STD Testing (collectively, “We,” “Us,” “Our”) collects and uses the personal information you provide on Our web site: https://www.prioritystdtesting.com/ (“Site”).

Collection and Use of Personal Information

We may collect certain identifying information from or about you in connection with your use of or submissions to Our Site, including, but not limited to, your name, address, date of birth, email addresses, health information, lab results, lab orders and credit card number (together, “Collected Information”). Any health information about you will be collected in the course of providing services to you via the Site and telephone consultations. In addition, We may retain the content of, and metadata regarding, any correspondence you may have with Us or Our representatives, regardless of the mode of communication by which such correspondence was made. This information helps Us to improve the Site and the online content, materials, and services that We make available on the Site, and to more effectively and efficiently respond to both current and future inquiries.

As with many other websites, the web servers used to operate the Site may collect certain data pertaining to you and the equipment and communications method that you use to access the Internet and Our Site and store it in log files. For security reasons and to confirm the integrity of the Site, We may combine components of these data with Collected Information, which may identify you. Such identifying information will be used solely for Our business purposes. Additionally, Our Site uses applications such as Google Maps to further assist Our patients. We may share Collected Information with the laboratory performing your test, PWNHealth and its affiliated professional entities, physicians and staff to provide its service to you. If all or part of Priority STD Testing is sold, merged, or otherwise transferred to another entity, We may transfer Collected Information to such entity as part of that transaction. You will be notified via email and/or a prominent notice on Our web Site of any change in ownership or uses of your Collected Information, as well as any choices you may have regarding your Collected Information.

We may disclose Collected Information to the proper authorities if We become subject to a subpoena or court order or if We are otherwise legally required to disclose such information. We may also use and disclose your Collected Information to establish or exercise Our legal rights, to enforce Our Terms of Service, to assert and defend against legal claims, or if We believe such disclosure is necessary to investigate, prevent, or take other action regarding actual or suspected illegal or fraudulent activities or potential threats to the physical safety or well-being of any person.

We are partnered with Trustpilot to encourage Our patients to review Our testing service.  Absolutely no medically related information is disclosed in Our automated review system, including the tests authorized, the status and results of the lab order, or any treatment consults with PWNHealth’s physicians.

What choices do you have?

When corresponding with Priority STD Testing or Our representatives, or when making a request for information or otherwise interacting with Us or others through the Site, you choose what information to supply, what questions or comments to submit, whether you wish to receive further information, and by what method of communication such information should be delivered. Please take care to share only such information as is needed or that you believe is appropriate. You are under no obligation to provide Us with personally identifiable information, but without it We may not be able to provide you the products or services you request.

How do We protect information collected about you?

Priority STD Testing maintains certain commercially reasonable security measures to protect Collected Information, both during transmission and once We receive it. When you enter sensitive information (such as credit card number) on Our Site, We encrypt the transmission of that information using secure socket layer technology (SSL). However, the storage and communication of Collected Information can never be completely secure. Hence We do not guarantee that information that you transmit or otherwise supply to Us, or any communications conducted on or through the Site, is or will be totally secure. Notice of Privacy Practices Regarding Health Information

This notice describes how medical information about you may be used and disclosed by Priority STD Testing and its partner PWNHealth, and how you can get access to this information. Please review it carefully.

How We May Use and Disclose your Health Information. We may use your health information and disclose it to appropriate persons, authorities and agencies, as allowed by federal and state law. Please be aware that state and federal law may have more requirements on how We use and disclose your health information. If there are specific, more restrictive requirements, even for some of the purposes listed above, We may not disclose your health information without your written permission as required by such laws. We may also be required by law to obtain your written permission to use and disclose your information related to treatment for a mental illness, developmental disability, or alcohol or drug abuse. We may do this without your written permission for the following limited purposes:

  3. Required by Law.
  4. Public Health.
  5. Reporting Victims of Abuse or Neglect.
  6. Health Care Oversight.
  7. Legal Proceedings & Law Enforcement.

Your Health Information Rights. You have the right to:

  1. Read and copy your health information.
  2. Request to correct your health information.
  3. Request to restrict certain uses and disclosures of your information. You have the right to request in writing that We restrict how your health information is used or disclosed. For most requests, under the law, We are not required to agree to your request. In some cases, We may not be able to agree to your request because We do not have a way to tell everyone who would need to know about the restriction. There are other instances in which We are not required to agree with your request. We will inform you when we cannot find a way to carry out your request.
  4. Receive a record of how We disclosed your information.
  5. Receive notification of a breach and obtain a paper copy of this notice.

 

California Privacy Rights Act (CPRA)

California Civil Code Section 1798.83, also known as the “Shine The Light” law, permits Our users who are California residents to request and obtain from Us, once a year and free of charge, information about categories of personal information (if any) We disclosed to third parties for direct marketing purposes and the names and addresses of all third parties with which We shared personal information in the immediate preceding calendar year. Right to request deletion of the data

You can ask for the deletion of your personal information. Verification process

Upon receiving your request, We will need to verify your identity to determine you are the same person about whom We have the information in Our system. These verification efforts require Us to ask you to provide information that We can match it with information you have previously provided Us. For instance, depending on the type o request you submit, We may ask you to provide certain information so that We can match the information you provide with the information We already have on file, or We may contact you through a secure communication method (e.g., phone or email) that you have previously provided to use. We may also use other verification methods as the circumstances dictate.

We will only use personal information provided in your request to verify your identity or authority to make the request. To the extent possible, We will avoid requesting additional information from you for the purposes of verification. However, if We cannot verify your identity from the information already maintained by Us, We may request that you provide additional information for the purposes of verifying your identity and for security or fraud-prevention purposes. We will delete such additional provided information as soon as We finish verifying you.

Other privacy rights

  • You may object to the processing of your personal information
  • You may request correction of your personal data if it is incorrect or no longer relevant, or to restrict the processing of the information.
  • You can designate an authorized agent to make a request under the CCPA on your behalf. We may deny a request from an authorized agent that does not submit proof that they have been validly authorized to act on your behalf in accordance with the CCPA.

Personal Information of Minors

Our Site is intended for individuals who are 18 years of age or older. We do not knowingly gather personal information from minors under the age of 18. If a minor under the age of 18 submits personal information to Our Site and We learn that it relates to a minor under the age of 18, We will attempt to delete that information as soon as possible. If you believe that We are in possession of personal information from a minor under the age of 18, please contact Us immediately.

New York (NY) Fee Disclosure

Testing services performed in New York (NY) are prescribed by physicians at PWNHealth.  Treatment services are also available through PWNHealth for any positive Gonorrhea, Chlamydia, and Trichomoniasis results. Patients in NY observe fees for the lab order, the ordering physician, and patient services. The total cost of testing is listed at checkout and billed as a one-time payment to “SFHT”.
